1. try to downgrade to IOS 4.1, and then download redsn0w and start the process from the scratch
2.if this won't do.. seems that you get a dead end :(

I already do all the process with Redsn0w; but i use IOS 4.1 instead of 4.2.1, by now my iPhone 3GS (week 25, BaseBand 06.15, bootloader 06.18) is workin' fine; BUT, yes there's a BUT...
i'm having issues with the signal strenght, sometimes i got 5 bars, sometimes 3 but mostly 4, in a full coverage place, but i can use the Phone... which is great
so.. if someone had the same issue or had any idea of what id could be, pleas post a comment

IF YOU DOESN'T MIND WARRANTIES!! READ FURTHER
by now the IOS 4.2.1 is only thetered, so i reccomend to stay away from update the Iphone to stock IOS 4.2.1
steps to follow:
download redsn0w, (i use a trick here... i used the IOS 4.1 instead of the IOS 4.2.1, so i got it unlockabe and unthetered ; X D); and you must choose the option 'use iPad's baseband' (the program fetch it from apple servers, so expect a big 'waiting time')
when everything is done, use ultrasn0w to unlock

if you use MAC use the PwnageTool to make a custom firmware (you need the iPad Fw 3.2.2 as well, in order to fetch the baseband (06.16), that's unlockable, and put into the IOS 4.2.1
(i use another trick here... i used the IOS 4.1 instead of the IOS 4.2.1, so i got it unlockabe and unthetered ; X D )
then you had to do a restore of the firmware using iTunes (using your fresh custom firmware)
then using ultrasno0w to unlock it
if you use PC then:

if you use a PC you had to download redsn0w, then choose the option 'use ipad baseband'... by now this thing needs intrenet, in order to fetch the ipad's baseband for the offical apple servers ; ), maybe later the dev-team will make a redn0w that doesn't need the online process, and can fetch the iPad's baseband directly for the fimrware donwloaded on your computer
