Well, three years to be exact, in fact... it WAS in the early days of pony. The fan fiction was posted on January the 30th 2011. My little pony began in 2010. So yes, I guess you could say it has been around 'forever'. And it has been popular. VERY popular, and I assume that you have never been on the Doctor Whooves side of the fandom before, so you really don't how popular it has or will be.

Okay, the whole Doctor Whooves and Derpy started back in the early days of pony. Doctor Whooves had just been discovered. Memes started floating around of him being 'My Little Timelord', and fan fictions as well. The first fan fiction of him was written by the one who discovered him, and Derpy happened to be the very pony who became his companion. A bunch of people liked this, and Doctor and Derpy became a thing. Pretty soon, Doctor Whooves and Assistant was born, and the pairing became more popular than ever. Other companions were offered to people of the fandom, like Lyra, or Twilight (even Apple Bloom) but few people accepted them as much as Derpy.
So, Derpy and Doctor is now what we call, a 'thing',
I hope that sums it up for you. (I've studied this kinda thing because Doctor Whooves is my favorite pony) XD
