Carriers do their testing and do an official release. The OS is then available on BlackBerry.com for the user to download. The user connects to BB Desktop and the phone gets updated with the new OS. BlackBerry also have regular OS "leaks", which are OSes released by BlackBerry (they carry RIM's digital signature countersigned by Verisign) but not yet released by Carriers.
