2.) I don't think you typically do, but it doesn't hurt. Just pop it in. Don't be scared, A sim card isn't going to make the 2G explode. Just make sure you tick "Activate iPhone" when you are about to build your ipsw.
3.) I haven't really seen this issue ever. I've been using the latest iTunes and not having to worry about it. Just toss it into DFU mode. If it does pull an error, the iPhone isn't bricked in the least. Just DFU again and try again. The only thing the latest iTunes will do is pull errors on unsigned ipcc files. I'm surprised the patched CommCenter, using MuscleNerd's patch, accepted my old T-mobile ipcc file on 3.1.2 (3G).
You'll be fine. iPhone 2G are practically failproof. At least all the software stress tests I've put mine through.

I'm sorry for those who "mistakenly updated" or "bought it that way" then you officially have an iPod Touch until the 5.xx.xx Baseband is broken. Sadly, asking how much longer doesn't make it come out any faster or IF it it can even be broken into.
On that note, this is a development community. Development is a slippery slope, you must read and do your homework to use their work. They warned you about updating, if you did you're sol. Sell the iPhone in an attempt to find one with a baseband of 4.xx.xx. Then read before updating. Be happy for everything they've given you for free, just don't berate them for "user error."
Everyone that is updating to 3.1+ I ask why per se? It is ridden with new signatures that take most the loved features away (carrier bundles, tethering etc.) I'm on iPhone 3G 3.0.1 on Tmobile USA. I went to 3.1 and it was horrible with lockouts and saw no new feature aside from arranging icons via itunes. The day I upgrade is the day they figure out carrier bundles and tethering. Til then, no dice. And if Appstore apps complain that they won't run on the older firmware just download FWchanger and mask it to 3.1.2.
Just my two cents, The dev team give gifts to us all, just to have their blog full of irate people demanding a fix for their mistakes.
