You normally get that error when you are, for example, using a PC normally, but JB'd your iPhone on a Mac. This is due to the different formatting options between Mac and PC, not anything to do with dev-team or Geohot.

Do the process via PC if you use a PC, or Mac if you use a Mac.

Google it, or search .ipsw using the belt in search function of your PC or Mac.

After downloading and installing ultrasn0w you must reboot your iPhone, and then it will be unlocked.

Are you sure ultrasn0w installed correctly?

You have to give more info for people to help you. What device are you using? What steps have you taken so far? Pwnage or Redsn0w, which version of Redsn0w, which .ipsw, Cydia or Icy?

Open Cydia, press Manage, press Sources, Edit, Add, addy, press right arrow, ultrasn0w

There is, as yet, no manual unlock (and it is unlikely there will be one). Ultrasn0w can only be installed through Cydia and Icy, over a wifi connection.
