Due to being possessed, Sam must now live with the memories of having almost raped Jo, having killed several people he didn't know (the hunter when possessed by Meg, the two angels when possessed by Gadreel) three of the people closest to him (Bobby and Cas when possessed by Lucifer and Kevin when possessed by Gadreel) and having almost killed his brother each time. Not even counting when he was soulless (which he has never forgiven himself for) Sam would never have agreed to possession, even to save his life, after the first two experiences. I always read it as him believing what he was doing (closing the gates of Hell forever) as the most important thing he could do. When Dean tells him that he will die if he continues, Sam just can't understand why that should make any difference. He is willing to give his life to save everyone that the demons could hurt in the future. In my opinion, that makes him heroic, not suicidal. Dean has no reason to see the offer as anything but legitimate. He doesn't know about the djinn collection or the attempt to capture the shapeshifter alpha. the first time I watched this episode, I thought Dean was being paranoid and his acceptance of Sam handing the baby to Samuel as his realizing that he was being paranoid and trying to tamp it down. Sam and Dean can't be the first hunters to have met "monsters" that didn't need killing. At least one shapeshifter that the boys met was at least in part shaped by the abuse in his childhood. If this baby was raised in an atmosphere of acceptance and love, there is no reason to think that it couldn't become a compassionate person. Who better to do that than hunters who would understand why it was different. But I do agree that Jensen's amazing ability to convey emotions truly marks and colors this scene! But his misunderstanding the intentions of others in the past (i.e. Lenore) made me give the Campbells the benefit of the doubt. Just like Bobby couldn't get out of the dream that Jeremy trapped him in until Dean convinced him it was a dream and helped him out, Dean was unable to get out of the dream (though he was able to control a portion of it, i.e. attacking his dream self) until Sam took out Jeremy.
