Remember Marlon Brando as Kurtz? Kurtz was supposed to be so thin as almost to be disembodied in Heart of Darkness, but in "Apocalypse Now" he's a lard-ass.

The point in the novel was to emphasize the disembodiment of the voice. I can't recall if critics made as big a deal about that, but I reckon not.
