2. T is above critical temperature, and pressure is less than circondenbar? Depends on the pressure and temperature, it could be vapor or multiphase.

If the fluid is above Cricondenbar and Cricondentherm then it is always in super critical phase. However if it is above Cricondenbar but below Cricondentherm themperature then it could be in liquid phase or more accurately fluid will have high densities similar to liquids.

As per the Code rules the internal hydrostatic test pressure is 1.3 times the design pressure or MAWP (with some temperature correction as applicable).

In past some code rules also required for test pressure to be 1.5 times the MAWP.

Note: MAWP should be same as design pressure.
