It seems like she was resistant to care and to a bipolar diagnosis, but I don't see where anyone asked her why. When people push back on meds I try to ask what the concerns are and allay them as best I can while coming up with a plan that will be helpful. The biggest red flag was the husband saying "my wife is not bipolar". There's a couple things I hope i would have done with that. The first, is something I do when people push back on a diagnosis which is whip out the dsm and compare their symptoms to the criteria. And explore why they don't think it's accurate. The second is that I hope I would have taken time at an appointment with just her to gauge what kind of support the husband is. And whether he is impeding her care or influencing what she says or reports. I get the feeling that he may have been minimizing her symptoms and making her feel a way about seeking help or taking meds. From what I can see from what I saw of the care she received looked like it was not negligent. In psych you are bound by what they tell you. You can't generally tell if someone is psychotic by looking at them and my experience is that people downplay those symptoms even when asked directly. Also you can look totally normal and functioning while having very bad delusions and hallucinations. You would be surprised how many people are walking around that you would never suspect. Also if her psychosis was related to lack of sleep, it will come and go. And the difference between intrusive thoughts and auditory hallucinations is academic. Also I totally understood what the doctor meant in that note where she used a comma instead of a conjunction.
