I understand the primal instinct to allow your children to be independent and adveturous, to prepare them for the harsh world. But there comes a time when you need to let that protective instinct take over, and when your child is out of your sight should be that time. It really isn't all that complicated. Don't let your child play with, fire, animals, guns, electricity, sharp objects, high places, out side, in the water, and other potentially dnagerous places/situations, unattended. YOU are the parent and YOU are the one who is supposed to be there to insure things like this don't happen not, wait until if/when it happens and then hope to deal with it. Now we have had our fair share of accidents in my house, but probly 90% could have been avoided with a little more supervision, and I feel very guilty about it every time. And I think that, is where the problem truely lies, not the fact that there was an accident, but the ownership of the cause.
