I'm a California licensed nursing home administrator; so this is my world that you're speaking about here. A world where too frequently too many people do not care as they should. In every nursing home I've ever been in or seen, CNAs are assigned specific patients-so clearly someone specific was at fault. But the rule of thumb in the world of nursing homes is that if you see a problem, and you're not already taking care of a patient with a problem as important or worse, then you take care of it. Common decency requires this. I would suggest a written note from you to the administrator of this facility. Make sure that if there's anything good you can say that you mention that, but the administrator needs to be aware that this type thing is going on so it can be addressed.
