I am going to take a wild stab at this. I can see phase tape so this is very close to the termination point. Therefore, I think that they have removed a piece of equipment in a demo. It is high voltage three phase, and I think, mind you wild stab here, that some brainiac maintenance man came up with this as a tap for a load monitor, or maybe to power a piece of equipment in the same enclosure. I don't have a lot in the pic to work with, but, if it was a thief tapping the conductors, there would be only one tap and a dead body. The circuit had to be open to do this, and guarded in an enclosure after.

#20 and #22 Are examples of a pretty common building practice. Those doors up high are for access for equipment to the second floor. If the architect foresees a need to bring things up to the second floor that a choke point elsewhere in the building might prevent. If you looked around you might find them at churches, warehouses, and schools. Any place where a short cut would prevent having to do major structural renovation. Most of the rest are obvious add ones from enthusiastic, if not qualified, amateurs. The remaining, well, illegal or unqualified labor, you get what you pay for. If you want a professional job, hire a professional to do it, and expect to pay for their skill and knowledge.
