A doctor, hospital, etc., signs a contract with an insurance carrier. Under that contract, they will be allowed to collect X number of dollars for procedure Y. It doesn't matter if they charge $100 or $100,000 dollars. If the agreed upon amount for that service is $50, then that is all that they can collect total between both the insurance company and the patient. The rest of the charged amount is written off. It is the insurance companies who set the amounts that we, and they, pay for services, not the providers.
