I worked for the VA for many years. Let me assure you from first hand experience that the bureaucrats operating that system have NOTHING BUT a profit motive in mind. All they EVER talked about was CREATING THE IMPRESSION of medical care being delivered so as to generate statistics to compete for a piece of the huge federal budget. In fact they knew nothing about science, health, ethics, or anything of real value. There were NO meetings to discuss patient care but countless meetings to discuss creating the appearance of treating patients. The bureaucrats knew nothing of American history, military history and most of them were ANTI-MILITARY and anti American. They were very Pro themselves and were great at building thick layers of bureaucratic insulation around themselves so that they could never be held accountable for anything. The VA has the greatest buck-passing tradition in the world; the one phrase you will never hear at the VA is, "I'll take care of that for you." Few of the bureaucrats knew anything about combat so you had the most naive people talking to combat soldiers and solemnly pronouncing cases of PTSD. The VA staff I worked with looked down on the soldiers or feared them. The soldiers were treated as victims of the nasty government. The bureaucrats knew nothing of commitment, valor, pride, loyalty, etc. The administrators were constantly trying to instill anxiety into all around them that their jobs were in jeopardy in order to control them. One of the favorite devices of the VA Bureaucrat was to send out a very officious memo signed by no individual but rather by some obscure department in the Bureaucracy. The VA would keep people in useless therapies for years to promote job security. Other veterans were invited to join "Research Studies" in which their regular medications were stopped and experimental ones substituted. Why was this done? Money of course: While each participating veteran was given a pittance the VA would collect $5-10,000 from the DRUG COMPANIES who needed the data on their new medications. Doctors drawn to the VA often sought out administrative positions or academic positions (in which they Treated NO patients while receiving a VA salary). The lowest tier of VA employees was the Physician/Clinician group. These people had no say in the operation of the VA Medical Center. There was a lot of secretiveness about the transfer of moneys between the VA and the University. Another great secret is the number of Malpractice complaints filed against the VA each year. No journalist I know has bothered to look into this and ask the right questions. I have actually been in meetings where the high level VA administrator said explicitly, ' What we need is an influx of new patients who have [one of several favored diagnoses] but who actually require no treatment.' If you have not worked in Government health care you just cannot imagine the ways in which federal government bureaucrats can screw up the best efforts of Doctors and Nurses to help the patients. You can always battle the GD insurance companies and often win. The federal government is a monster answerable to no one. I know whereof I speak. The patient insisted he knew that the Doctors were all greedy businessmen who were performing unnecessary treatments on the hapless patients. He apparently wanted to be allowed to enter a new executive position of his own design where he could tell both the Doctors and the patients what to do. He claimed that the patients there and others outside the Hospital had communicated to him that he must do this. When asked how he came to possess this knowledge he stated that it was a commonly accepted fact. When the Interviewer asked him what qualifications he held for such a position, he said he had consulted with Thought Leaders from both sides of the intellectual spectrum, from Lawyers to Community Organizers. When asked what Hemoglobin was he produced a convoluted argument that led to the conclusion that the U.S. Constitution was “flawed.” Further attempts to discuss a factual basis for his ideas were met with increasing indignation and the adoption of a contemptuous demeanor apparently meant to discourage the Interviewer. He claimed to be able discern massive inefficiencies and conspiracies which he needed to correct immediately. He said that he had come to understand from his research that much Money was being wasted trying to keep people alive who were boring and produced no tax revenue. These "former people, he said, "were subject to dementia in which they believed the government should not hasten their death deliberately using the tax dollars collected from them." He claimed to have invented something called “Evidence Based Medicine” because he learned in Law School that Doctors made decisions on the basis of whimsy, or avarice, or impulse. Interestingly, when asked to interpret proverbs he was able to abstract eloquently, although tangentially, but was unable to give a simple concrete statement as to what the proverb actually said (the reverse of the usual Psychotic presentation). A request to do a simple calculation produced an answer of 47 million. The patient was asked about depression and denied all symptoms except that he readily endorsed continual gloomy paranoid ruminations and revenge fantasies against someone named “Whitey.” Although admitting he had never seen this individual he said he could feel his menacing presence everywhere throughout his life. He claimed that “everyone” had corroborated his experiences with “Whitey” except Whitey’s friends. Thought broadcasting was evident from his belief that he could craft phrases that would control the minds of others. He spoke frequently of his ability to “calibrate” his statements so that there would not be, could not be, any opposition except by Racists. When asked about his sexual life he said he was mainly excited by hearing himself calibrate his own utterances to peak effect. He said he had reached orgasm on several occasions while watching himself on TV and asked if this was abnormal. He insisted there could be no other point of view but his own and although the word “empathy” was sprinkled in his verbiage he could not describe the feelings of a single person who disagreed with him. His desperate attempt to maintain a posture of self adulation was obviously a defense against deep feelings of inferiority and self loathing. He was impossible to interrupt and totally devoid of insight into his mental illness. His judgment was grossly impaired by his grandiose, paranoid, sadistic, and megalomaniacal delusional system. He insisted he owned an automobile company and many banks although admitted he had no money. He laughed when asked if he thought he could ever be manipulated by others.
